ON, Toronto, Oakville, Richmond Hill – IL Fornello

From Karen at Avoiding Milk Protein, "I talked to a waitress who was anaphylactic to both milk and nuts and eats regularly with out issues."
It seems IL Fornello offers dairy and gluten free menu items amongst their pizzas and pastas.  See their website (below) for the menu (dairy-free might be cheeseless, I am not sure) and other vitals, such as locations, phone numbers, and hours.  In general, they are open for lunch and dinner.

An additional note from Karen, "… they have an alternative menu with vegan soy cheese, and gluten free pastas and pizza breads."

They have several locations in Toronto, plus one in Richmond Hill, and one in Oakville.
